All new and transfer students are required to submit complete and accurate health information about their overall health and vaccination/immunization history. Student Health Forms must be submitted and complete by the stated deadline. The list of all required forms is located on the Student Health Services Portal.

Health Form Due Dates:
Fall Semester – August 1
Spring Semester – January 1

The Farmville Family Pharmacy, an independent pharmacy located in Farmville, offers students the opportunity to set up an individual account. They will

• allow the student to receive their prescription without paying at the time of pickup
• make deliveries to the campus on Monday, Wednesday and Friday

Additionally, if your son needs over-the-counter medication or supplies that are stocked in the pharmacy, they can be delivered to campus. Students may either arrange a drop-off location or pick up their orders at the Health Center.

This program is maintained by the Pharmacy. If you are interested in setting up an account, download the Farmville Family Pharmacy Intake Form, and submit it directly to Farmville Family Pharmacy. Contact information is on the form.  Please call the Pharmacy if you have further questions.

Student Health Services staff believes that:

• Each student has the right to treatment with dignity by a professional medical staff that has experience in the care of college students.
• The information that a student shares with us must be kept in confidence; it will not be shared without the student's permission.
• A student should be given the opportunity to be a partner in his care and should be given choice of treatment options and of health care providers when such options are available.
• A student should be given accurate, clear information regarding his health problem and prescribed treatment.
• Students have the responsibility to be truthful about their concerns, to follow their prescribed treatment plan, and to ask questions.
